Treat Shoulder Pain after Motor Vehicle Accident – Columbus, Ohio

Patient: 25 year old male, 5’-10”, 170lbs, blood pressure 126/84mmHg, pulse 72bpm, and 14 breaths per minute.

Mechanism of Injury: The victim was involved in a head on collision car accident at 30mph. He did not go to the hospital.

Subjective: He reports severe right shoulder pain and reports it is getting worse. He presents to the office with tingling in his fingers and has difficulty using his right arm for daily activities with pain rated 7/10.

Visual Inspection: His right shoulder was elevated and internally rotated.

Manual Palpation: He had severe muscle guarding in his pectoralis minor and posterior deltoid. There was mild swelling of his right rotator cuff muscles.

Range of Motion Assessment: He could not reach more than 20 degrees in front, 45 degrees to the side, and 5 degrees backward.

Shoulder X-Rays: Bone density is adequate with no evidence of fracture. There is evidence of soft tissue swelling between the supraspinatus and humerus.  The acromioclavicular (AC) joint was increased and was rated as a grade 1 separation.

Chiropractic Treatment Day 1: After treatment with a cold pack and vibration therapy, and a soft adjustment, his right shoulder pain decreased to 5/10 and could bring his arm forward 70 degrees, backward 20 degrees, and to the side 80 degrees before discomfort set in.

After 1 Week of Treatment: His right shoulder pain was a 3/10 and he was able to drive and use his arm for daily activities again.

The patient was released on the 4th week of treatment due to improved mobility, strength, and reduced pain to 0/10. His blood pressure was 116/72mmHg.
