Patient: 18 year old male, 5’-11”, 165lbs, blood pressure 122/84mmHg, pulse 62bpm, and 16 breaths per minute.

Mechanism of Injury: The driver was involved in a car accident and was hit rear ended from a dead stop by a vehicle driving 45mph. He did not go to the hospital.

Subjective: He had extreme discomfort a few hours after the crash and reports it is getting worse. He presents to the office with a sharp, throbbing, stiff pain rated 9/10 in the middle of his back.

Visual Inspection: His shoulder was elevated on the right and his right hip was also elevated. There was no bruising or swelling noted.

Manual Palpation: He had severe thoracic paraspinal muscle spasms in both of his rhomboids and left levator scapulae. He had muscle guarding of the thoracic paraspinal extensors.

Range of Motion Assessment: He could not lean to the left and could not rotate his torso without extreme discomfort. His pain worsens when he breathes, coughs, or sneezes.

Thoracic X-Rays: Bone density is adequate with no evidence of fracture. There is no evidence of soft tissue swelling. There is a left lateral lean from T2-T6.  There is decreased disc height between T4/T5.

PATIENT TESTIMONIAL

Chiropractic Treatment Day 1: After lying on an ice pack, vibration and traction table for 10 minutes, his back pain decreased to 6/10 he could lean to his left a little more. Coughing still aggravated his condition with mild discomfort, less than half of what it was before treatment.

After 1 Week of Treatment: His back pain was a 4/10 and he was able to breathe, cough, sneeze, and lean to the left 20 degrees without pain.

The patient was released on the 3rd week of treatment due to improved mobility, strength, and reduced pain to 0/10. His blood pressure was 120/78mmHg.
